- Initial post-discharge nursing assessment and care-plan review

- Vital-sign monitoring and symptom checks

- Pain, nausea, hydration, nutrition, mobility, and sleep support

- Medication reminders and medication-schedule organization

- Incision, dressing, drain, and wound observation within nursing scope and ordered plan of care

- Assistance with ambulation, repositioning, toileting, bathing, and activities of daily living

- Compression garment, binder, ice-pack, and positioning assistance

- Incentive spirometer reminders and pulmonary hygiene support

- Intake/output tracking and bowel-regimen support

- Red-flag education and escalation to surgeon, provider, emergency services, or caregiver when appropriate

- Appointment preparation, discharge-instruction reinforcement, and recovery documentation

- Family/caregiver education and handoff
